About Us EMPower Eldercare Management EMPower’s team includes a masters level counselor and registered nurse who conduct comprehensive reviews of physical systems, functional ability, medication compliance and psycho-social evaluation and also create, implement and monitor short and long term plans. Each of our professionals are members of the National Association of Professional Geriatric Care Managers.
Meet Our Team
An entrepreneur, Lynn founded and directed her own rehabilitation counseling and consulting firm, Cox & Associates where for 10 years, she successfully coordinated employees, employers, physicians, attorneys, work capacity evaluators and insurance claims adjusters to facilitate returning the industrially injured to work. On many occasions during court hearings, Lynn provided expert testimony regarding disability management and rehabilitation issues of the injured worker and she is certified as an expert in Federal Court, Sacramento. After experiencing the lengthy decline of a close family member due to Alzheimer’s disease, Lynn developed a passion to serve the senior population. Her wide range of experience working with older adults and their families in meeting their short term and long term care arrangements through personalized care planning and dedication to serving the older adult has led to her launching of EMPower Eldercare Management. Lynn is a member of the National Association of Professional Geriatric Care Managers and the American Society on Aging.
Diane is a Certified Professional Disability Manager with over 15 years experience in the insurance and disability management arena. As a claims manager, her responsibilities included; facilitation and resolution of insurance claim issues, while functioning as a liaison with physicians, medical case management nurses, families and insurance carriers. Diane contributes to EMPower Eldercare Management as our specialist in medical insurance claims, benefit programs, bill-paying/banking, organization of tax materials and residence transition/moving assistance. Diane also contributes personal understanding of the aging process as she is currently caring for elder loved ones in her family. Diane is a member of the National Association of Professional Geriatric Care Managers and a member of the American Association of Daily Money Managers.
With a career that began in geriatric nursing, Dana brings vast skilled, acute, critical and outpatient nursing care and management experience to EMPower. A certified IV Specialist, she performed over 500 peripherally inserted central catheters in her 17 year career at Davies Medical Center in San Francisco where she was instrumental in founding, developing and overseeing the first outpatient infusion service clinic in the United States specializing in AIDS/HIV patient care. During this period, Dana went from working with 2-3 patients daily to 900 a month. Earning her Chemo certification to service cancer patients, Dana also received her Research Nurse Certification, working with investigational drug studies. Dana is a recipient of the prestigious San Francisco Cable Car Award. Dana is a member of the California Board of Registered Nurses, National Association of Professional Geriatric Care Managers, and a member of the American Association of Daily Money Managers.

Gail graduated from Stanford University with dual degrees as Family Nurse Practitioner and Physician Assistant. Gail has years of experience working with older adults with complex medical conditions within the hospital and home health care environments. Currently Gail is a Trauma Nurse Practitioner with San Francisco General Hospital where she performs management of acutely admitted trauma patients.
Prudence is currently Director of Care Management at San Francisco Health Plan, a California Managed Care Organization serving underprivileged populations in government-sponsored programs. She oversees all case management programs including disease management programs for Type II Diabetes, Chronic Pain, and Weight Management and Obesity. She has over 15 years experience in Case Management and Utilization Management that encompasses commercial, Medicare and Medi-Cal environments. She previously managed Western regional operations for UM and CM for a large national health insurance company. Prior to stepping into managed care, Prudence practiced as a Registered Nurse in various hospital acute care settings, specializing in adult oncology. Her clinical background includes work in acute care, rehabilitation and skilled nursing settings.
Michael earned a Master of Science in Physical Therapy from University of California, San Francisco. Michael specializes in work with geriatric clients in home or community based rehabilitation programs; assessing and treating strength, endurance and mobility needs, safety of home set up and fall risks, equipment and adaptive device needs as well as family and caregiver training.
