Key Benefits:

• EMPower care managers will develop a plan of care with specific recommendations for services to meet the needs identified in the assessment as well as arrange and coordinate home, nursing, community services or facility care (in conjunction with family members whenever possible).
  
  
• EMPower care managers will ensure that Medicare, other insurance coverage, and/or publicly financed services are used to the extent that the insured person may be eligible for such services.
  
  
• EMPower care managers will counsel and educate family caregivers and help maximize informal supports.
  
  
• EMPower care managers will provide information and referral services and determine which community based resources might be available to get the most cost-effective use of the benefits and receive care in the most appropriate setting.
